Your First Appointment    

After making an appointment with Mary Eshbaugh (for Janis Carney) or Linda Fletcher (for Leslie Yarnes Sugai), please take a moment to inquire what information should be brought with you to that initial consultation, in view of the particular matter being addressed. Organizing pertinent papers and documents ahead of time will increase the productivity of your meeting.

When you arrive at our offices, you will be given a pink information sheet and asked to fill it out completely. You will also be given a form explaining our fees. The information sheet will be used to provide initial information for the attorney who will consult with you and eventually as a basis for opening your file. If you decide to retain Carney & Sugai to provide legal services beyond the initial consultation, you will be asked to sign a Retainer Agreement outlining the services to be performed and the fees to be charged. Naturally, your signature constitutes agreement to the services and fees therein.

Estate planning clients will want to download and print both the Confidential Estate Planning Questionnaire and the Memorandum to Client Regarding Advance Health Care. Filling both forms out completely and bringing them with you to the first meeting will allow our attorneys to draft your estate planning documents in a timely manner.

Clients seeking representation by Carney & Sugai as a couple will need to download and print one of the two Consent for Dual Representation forms in order to fill it out, sign it, and bring it with them to their first meeting. Mary or Linda will be happy to let you know which form is appropriate for your situation.

Those who require assistance with Medi-Cal planning should download, print, and fill out the Medi-Cal Planning form, bringing it to the first appointment. All who plan to file for conservatorship will want to download, print, and complete the Conservatorship form to bring with them.
